Indications/Uses
Generalized epilepsy particularly w/ absence, myoclonic, tonic-clonic, atonic & mixed patterns of seizures, & partial epilepsy particularly w/ simple or complex, secondary generalized, specific syndromes (Lennox-Gastaut); treatment & prevention of mania associated w/ bipolar disorders.
Dosage/Direction for Use
Epilepsy Adult 600 mg in 2 divided doses. May be increased by 200 mg every 3 days to 1-2 g daily (20-30 mg/kg daily) up to a max of 2.5 g/day. Childn 20-30 mg/kg/daily, may be increased to 40 mg/kg daily in severe cases. Combination w/ anticonvulsants (phenytoin, phenobarb & carbamazepine): Initiation therapy should be gradual to raise by 5-10 mg/kg/day. Bipolar Initially, 20 mg/kg/day. Max: 3,000 mg/day. Maintenance: 1,000-2,000 mg/day.
Administration
Should be taken with food: Swallow whole, do not chew/crush.
Contraindications
Hypersensitivity. Acute & chronic hepatitis, family history of severe hepatitis, drug-induced hepatitis, porphyria. Patients known to have mitochondrial disorders caused by mutations.
Special Precautions
Liver damage. Renal insufficiency. Cell & platelet count, bleeding time & coagulation test are recommended before initiating therapy & surgery. Potential risk in patients w/ SLE. Hyperammonemia. Hypothermia. False interpretation of urine ketone test. Monitor signs of suicidal ideation & behaviours. Women w/ childbearing potential. Pregnancy & lactation. Childn <3 yr & infants.
Adverse Reactions
Confusion or convulsion; GI disturbances eg, nausea, gastric pain; hair loss, fine postural tremor; wt gain, amenorrhea, menstrual disturbances; dose-related rise in bleeding time, decrease fibrinogen; thrombocytopenia; isolated hyperammonaemia.
Drug Interactions
Generalized convulsions w/ imipramine antidepressants. Increased plasma conc of phenobarb & zidovudine. Decrease plasma conc of phenytoin. Increased plasma levels w/ enhanced adverse effects (sedation) of primidone. Clinical toxicity w/ carbamazepine. Reduced metabolism of lamotrigine. Decrease clearance of felbamate. Increased metabolism w/ mefloquine & chloroquine. Reduction in serum conc of w/ carbapenems. Decreased absorption w/ colestyramine. Decreased blood levels w/ rifampicin. Risk of encephalopathy &/or hyperammonaemia w/ topiramate. Increased free serum level w/ aspirin, cimetidine, erythromycin, vit K.
